09Built-in EP stimulatorPace the HRA, RV, RV base or para-Hisian region with S1–S4 extrastimuli on a sweeping 13-channel recording (surface ECG, HRA, His, decapolar CS, RVA). Induce and terminate AVNRT and AVRT, find Wenckebach and refractory periods, and watch every beat conduct through the 3D heart with the catheters in place.
10Case challengeAn unknown tachycardia runs with its name hidden: AVNRT, orthodromic or antidromic AVRT, focal AT, typical or atypical flutter, or VT. Work it up like in the lab (entrain from the RV and the atria, read PPI − TCL and the V-A-V or V-A-A-V response), commit to a diagnosis, then see the answer and what each maneuver showed.
